Membership with AHLTA is free and open to transplant recipients, families and carers across Australia. It only takes a few minutes to join.

Application form

We invite you to become a member of the Australian Heart and Lung Transplants Association (AHLTA).

AHLTA is a registered Australian charity, founded in 1984 with the encouragement of Dr Victor Chang, to support people before and after heart and lung transplantation, as well as their families and carers. Find out more about AHLTA here.


Who can join

  • Anyone who has undergone a heart, lung, or heart–lung transplant in Australia may become a Full Member.

  • Patients currently waiting for transplant may join as Associate Members.

  • Carers, friends, parents and guardians of transplants recipients may join as Associate Members.

Membership is free. By joining, you will receive an AHLTA membership badge and ongoing information about our activities, events and support programs.

If you are a parent, guardian or carer please complete the whole form with your details and provide more information about the transplant recipient when when prompted.
